Business registration form
Joining the Carer Business Discount Card scheme is easy and free. The quickest way to join is to register online. You can also choose to join the Seniors Business Discount Card scheme.
Alternatively, download the registration form, complete your details and return to Card Services. If you prefer you can speak directly with Card Services about your registration.
Before completing the form you should read the terms and conditions and decide on a discount or incentive offer that will be valuable to a Carer Business Discount Card holder. Examples of offers include:
- a flat rate percentage discount (e.g. 20% discount)
- special low prices just for cardholders
- a free service (e.g. home delivery).
Remember, the better the offer the more people will be encouraged to use your business.
More information about the Carer Business Discount Card scheme and promotional material will be sent to you once your application to register has been processed.
Terms and conditions
The Carer Business Discount Card scheme and the Seniors Business Discount Card scheme (‘the scheme’) are administered by Card Services, Smart Service Queensland, Department of Communities, Queensland (‘the Department’).
Businesses registered with the Carer Business Discount Card scheme agree to provide discounts or offers to holders of a Carer Business Discount Card.
Businesses registered with the Seniors Business Discount Card scheme agree to provide discounts or offers to holders of a Seniors Card and Seniors Business Discount Card.
In registering with the Carer Business Discount Card scheme or the Seniors Business Discount Card scheme, or both the business provider:
- accepts responsibility for and warrants it will ensure the offer will be readily available to nominated cardholders;
- agrees to display the logos in a prominent place in all business outlets that participate in the agreed offer;
- shall not represent the Department as the provider or supplier of the offer;
- acknowledges the Department makes no representation that any minimum number of sales will result from participation in the scheme;
- warrants to the Department that products or services sold shall be free from defects in materials or workmanship and will be fit for the purposes for which the offer is purchased by the customer;
- agrees the offer may not be changed or substituted without the prior written consent of the Department;
- agrees to keep the Department fully indemnified against any costs (including legal fees), claims, damages and expenses that may be incurred by the Department as a result of the business provider failing to perform its obligations under this agreement and/or in connection with the supply, or any failure to supply, the offer;
- consents to the making of enquiries and the exchange of information with the Office of Fair Trading and other authorities regarding its trade activities and any other matters relevant to this application;
- agrees business registration in the scheme ceases when the offer is withdrawn or is no longer available to cardholders;
- agrees to immediately remove all logos from display and remove or withdraw all advertising material containing references to the scheme, when business registration in the scheme ceases;
- agrees business registration may be withdrawn by the Department where the business provider fails to comply with relevant standards and guidelines set by the Department as appropriate to the industry or business;
- acknowledges the Department can refuse, at its discretion, to admit applicants to the scheme;
- agrees to contact the Department when wishing to withdraw from the scheme;
- acknowledges the Department will advise cardholders to take any unresolved business disputes to the Office of Fair Trading;
- agrees registration of the business under the scheme does not constitute an endorsement by the Department and will not in any way hold out that the products or services of the business are endorsed by the Department.
Privacy statement
All business information may be published in Smart Service Queensland publications or on the Seniors Card and Carer Business Discount Card websites and is subject to false and misleading advertising provisions of the Fair Trading Act 1989. The Department of Communities is collecting information on this form to assist in the administration of the Seniors Business Discount Card scheme and Carer Business Discount Card scheme.

Smart Service Queensland, Department of Communities may confirm business information with the Office of Fair Trading to assist with assessment of your registration. Some business information may be accessed by the public through Queensland Government websites and business information may also be provided to a cardholder in response to a query.
