How to apply
All applications must be lodged on an official application form. Application packs, including an application form, guidelines for completing an application form and frequently asked questions, may be obtained by phoning:

Alternatively, download an application kit including frequently asked questions, application guidelines (including a full list of the Queensland institutions covered by the Redress Scheme on page 14), and the application form:
All applicants must:
- read the frequently asked questions and application guidelines
- check the list of facilities to ensure they are eligible
- complete the application form
- mark completed applications as 'PRIVATE & CONFIDENTIAL' and submit to:
