Apply for a Carer Business Discount Card

There are several ways to apply for a Carer Business Discount Card. The quickest way is to apply online.

Alternatively, download the cardholder application form (PDF), complete your details and return to Card Services.

If you prefer, you can make your application over the phone by calling Smart Service Queensland on 13 QGOV (13 7468) (Monday to Friday, 8.00 am - 6.00 pm). Remember to have your Centrelink Customer Reference Number (CRN) handy.

Who can apply?

To be eligible for a Carer Business Discount Card you must:

• be a resident of Queensland with a Queensland residential address registered with Centrelink, and
• receive the Centrelink Carer Payment or Carer Allowance.

What if my circumstances change?

You must notify Smart Service Queensland on 13 QGOV (13 7468) (Monday to Friday from 8.00am to 6.00pm) of any changes to your circumstances as soon as possible, including:

• changes to your name or address
• if you no longer receive the Centrelink Carer Payment or Carer Allowance
• loss of card.

Any changes to your name or address must be registered with Centrelink first. Your card eligibility is linked to the personal details registered with Centrelink and will be confirmed by the Carer Business Discount Card scheme.

Once you have contacted Centrelink with any changes, you can manage your card online (e.g. request a replacement card, update your details, or cancel a card).

Cardholder Agreement and Authorisation

• I understand that my personal details are protected by the Information Privacy Act 2009.*
• I understand that Card Services, Smart Service Queensland, on behalf of the Department of Communities, Child Safety and Disability Services, will confirm my details held by Centrelink to verify or confirm my eligibility.
• I will advise Card Services, Smart Service Queensland of any changes in my circumstances that may affect my eligibility for a Carer Business Discount Card.
• I understand that if I am in possession of a Seniors Card or Seniors Business Discount Card, Card Services, Smart Service Queensland will automatically update my current details for these cards.
• I understand Card Services, Smart Service Queensland on behalf of the Department of Communities, Child Safety and Disability Services or their authorised agent/s (e.g. University) may use my details to contact me to undertake research, evaluation or review (e.g. voluntary survey/questionnaire) to ensure continuous improvement in the Carer Business Discount Card scheme.
• I understand that I can revoke this consent or cancel my card at any time by writing to Card Services, Smart Service Queensland.
