Home and Community Care Home Modification Services are funded through the joint Commonwealth/State Home and Community Care Program and delivered by the department.
Eligibility is determined by Home and Community Care Program National Guidelines. The service is for older and frail people or people with moderate, severe or profound disabilities, who need support with home modifications to prevent premature or inappropriate long-term residential care in a hostel, nursing home or similar institution.
The services provide information, assessment, project management and/or financial assistance for larger home modifications, such as ramps and bathroom adaptations.
