Apply for a card

There are three ways to apply for a Seniors Card, Seniors Card +go or a Seniors Business Discount Card:

  • complete the online application form. Remember, to lodge your application electronically, you need to click on the 'Submit' button at the bottom of the form when you are finished
  • in person at any QGAP office or at the Queensland Government Service Centre, 33 Charlotte Street, Brisbane
  • complete a hardcopy of the application form. Contact Smart Service Queensland on 13 QGOV (13 7468) during business hours for a copy of the form. You can post the completed form with a photocopy of your proof of age*, to:

* Accepted forms of proof of age include a Centrelink or Department of Veterans' Affairs number, your birth certificate, a current Australian passport (or an overseas passport with a permanent visa), Queensland driver's licence, or a Queensland 18 Plus card. Proof of age documents MUST match the name used on the application form.

  • If you do not have any of these documents, you will need to provide a Statutory Declaration (PDF) signed by a Justice of the Peace stating your name, address, date and place of birth.

Convert to or from a Seniors Card +go

If you hold a current Seniors Card you can convert your card to a Seniors Card +go by completing the form online or call Smart Service Queensland on 13 QGOV (13 7468) during business hours. (This applies only to current Seniors Card holders, not Seniors Business Discount Card holders).

Replace a card

If your card has been lost, stolen or damaged, you can replace a card by completing the form online or contacting Smart Service Queensland on 13 QGOV (13 7468) during business hours to request a replacement card.

Change card details

If you have changed your postal address, email address or phone number, change card details by completing the form online or call Smart Service Queensland on 13 QGOV (13 7468) during business hours to update your  details.

If you have changed your name you need to provide us with supporting documentation, such as a photocopy of a marriage certificate, driver's licence or divorce decree. To be issued with a new card, simply send a photocopy of one of these documents, along with your old card, to:

Upgrade a Seniors Business Discount Card to a Seniors Card or a Seniors Card +go

You can upgrade a Seniors Business Discount Card to a Seniors Card or a Seniors Card +go by completing the form online or contacting Smart Service Queensland on 13 QGOV (13 7468) during business hours.

To be eligible for a Seniors Card, you must be:

  • 65 or older and have recently stopped working full-time, or
  • between 60 and 64, not working full-time, and have recently received a concession card from Centrelink or Veterans' Affairs.

Cancel a card

If you need to cancel an existing card for any reason (for example, you have moved interstate or the card holder is deceased) you can cancel a card by completing the form online or call Smart Service Queensland on 13 QGOV (13 7468) during business hours.
