This form is a mandatory requirement. A completed copy of this form must be sent to the Queensland State Award Office at least one week prior to the AJ activity.  Please also ensure each participant completes a 'P4 Adventurous Journey Consent Form' which can be found below.

Supplementary

These forms have been developed for your use. You are not required to submit these to the Queensland State Award Office and your organisation's versions of these forms can be used instead.
