Forms
In order to carry out the screening process, the Department of Communities will collect information through a range of forms. The Act outlines a number of approved forms for the collection of information to be used for a number of different purposes related to criminal history.
Application for criminal history screening – prescribed notice (yellow card) (Form 10-1)
Used each time an application is made by a non-government disability service provider funded by the Department of Communities for a criminal history check on a person engaged or about to be engaged by the organisation. Also used for notification to the department for volunteers transferring to paid employment.
Application for criminal history screening change of details (Form 10-2)
Used by a person engaged or seeking to be engaged by a non-government disability service provider funded by the Department of Communities.
Application for replacement positive notice card (yellow card) or positive exemption notice and card (Form 10-3)
For use by a person engaged by a non-government disability service provider funded by the Department of Communities whose positive notice, positive notice card, exemption notice or exemption notice card has been lost or stolen.
Change of details of positive notice (yellow card) or positive exemption notice holder (Form 10-4)
For use by a person holding a positive notice or positive exemption notice who changes their name, starts to use a different name or changes their contact details.
Application for cancellation of negative notice or negative exemption notice and issue of positive notice or positive exemption notice (Form 10-5)
For use by a person seeking to have a negative notice or negative exemption notice cancelled and to be issued with a positive notice or positive exemption notice.
Application for information from register of engaged persons (Form 10-6)
For use by a non-government service provider funded by Disability and Community Care Services requiring information about the current status of an application for criminal history screening or any positive notice/positive exemption notice or negative notice/negative exemption notice issued.
Application for eligibility declaration (Form 10-10)
For use by a disqualified person seeking to be declared eligible to apply for a prescribed notice.
Application for criminal history screening (exemption notice) (Form 10-11)
For use by a Department of Communities funded non-government disability service provider on behalf of a blue card holder engaged or seeking to be engaged by the service provider.
