In establishing continuity and consistency of services throughout the transition period and beyond, we will work together with stakeholders and non-government organisations. This is in addition to our collaborative work with the Commonwealth and other Queensland Government agencies, to implement the changes to aged care and disability services as outlined in the new National Health Reform Agreement.
