  • How to contact Disability Services and request support

    You can contact Disability Services in a number of ways, but the easiest and most convenient method is by telephone. You can call Disability Services free of charge through Smart Service Queensland on 1800 177 120*, or you can phone your local Disability Services service centre direct.

  • Notification about compensation relating to disability - Form 15

    For use by, or on behalf of, a person with a disability, who has received, or may receive, an amount relating to the disability.

Portability Information

Department of Communities, Disability and Community Care Services, will consider requests to transfer funds allocated through its various funding initiatives between service providers within Queensland as well as between State and Territories.
