The Request for Assistance Form is one of the ways you can contact Disability Services to request specialist disability support.
You do not have to fill out the Request for Assistance form. You may choose to simply phone Disability Services to request support, and a trained intake officer will ask you a series of questions to gather the information they need. They will also be able to answer any questions you might have about the process.
You can complete the Request for Assistance Form if you prefer not to contact Disability Services directly.
The Request for Assistance Form has been specially designed to make access to specialist support simpler and easier and contains just three pages for you, or a person on your behalf to complete. A list of service centres where you can lodge your request for assistance are appended to the end of the form.
