Remote service delivery: Gold Coast, Brisbane, Fraser Coast, and South Burnett.

Target group

All people with a disability from 18 to 65

Families of children with a disability from 16 to 18 transitioning from school.

Organisational Information

121 Care is a not-for-profit organisation that has been providing flexible, individualised, in home support to people with disabilities for over 20 years throughout South East Queensland from Brisbane to the Fraser Coast. The organisation was established by people with disabilities for people with disabilities and it empowers its clients to live and manage the lifestyle of their own choice. Our services include:

The 121 Care staff work closely with you, your family and workers as an integrated team creating and delivering a unique lifestyle for you that meets your individual needs, your personal philosophy and your community participation requirements.

We achieve this individualised, tailored approach by developing long term relationships with you and your family. This includes open and honest communication, empathetic listening and providing you with a variety of care and lifestyle options for you to make your choices. We pride ourselves in delivering innovative and unusual solutions to everyday problems.

Host Provider Model

121 Care provides a spectrum of services using its unique MyChoice™ model which reflects the processes used by lifestyle planners to capture your dreams, goals and desires and then develop an action support plan to create your desired reality.

This program provides you with a broad variety of services and suppliers which are supported by 121 Care, its partners and other regional service providers. You can then develop the lifestyle of your choice depending on the level of responsibility, control and participation that you are comfortable with. As part of the MyChoice™ model, 121 Care provides a series of seminars as well as one on one support and training to assist you to increase your skills and knowledge to successfully manage your own service supports and funding.

You can choose the particular services that you require to manage your life and we will assist you in locating and negotiating those services with the suppliers of your choice.
121 Care's 6 step MyChoice™ process covers the following steps:

C – Choose the lifestyle you want using our specially designed Lifestyle Fact Find
H – Host provider, choose who you want to manage your funding
O – Options, choose the specific types of services and supports you need to manage your individual lifestyle
I – Identify any training you need to assist you to self direct and implement your choice of services through your purchases
C – Control your plan and your life using report structures, regular financial statements, responsibility descriptions and the feedback and complaints process
E – Evaluate and review your lifestyle plan 12 monthly or as necessary and review the plan to ensure it continues to meet your needs.

Staffing management services

  • Staff recruitment and reference checking
  • Security, licence and training checks
  • Staff induction and training
  • Performance management of staff
  • Termination of staff
  • Payroll services
  • Payment of government liabilities such as PAYG Tax, Superannuation and Workcover

Fee Structure

Initial meeting

When we first meet you we spend time with you and your family to determine your lifestyle needs and requirements to determine if we are the most appropriate resource organisation for you. This meeting is free of charge so that you can get to know us and we can get to know you.

MyChoice™ Lifestyle Fact Find - $575 (includes financial establishment fee)

  • Fact find meeting identifying your support levels, lifestyle goals, service needs, equipment needs and level of responsibility for managing the plan
  • MyChoice™ lifestyle plan development
  • Budget preparation

MyChoice™ 12 Monthly Review – Free of charge

Hosting and Administration - 9% of funding charged monthly

  • Hold and manage your funds
  • Pay accounts as authorised by you based on your plan
  • Provide quarterly funding and expenditure statements to you
  • Develop and monitor your own budget
  • Provide training in budget preparation and management
  • Provide budget tools and applications to assist you in managing your funds
  • Report to the government on the your behalf
  • Contract management and reporting

Exit fee - $150

An exit fee will be charged when a participant exits 121 Care to move to a different host provider in order to cover administration and handover costs.
