All people with a disability aged 0 – 65 years and their families/carers.

Organisational Information

Lifestyle Solutions Queensland support services focus on two key areas;

  • Disability Support
  • Child and Family Services

Under these support streams we have a range of flexible and responsive programs designed to achieve positive outcomes that focus on living skills, social inclusion and community participation.

Our flexible approach to the provision of support means that we can adapt our service to better suit individuals as their requirements evolve. At all times we focus on:

  • Promoting active inclusion and participation in community life
  • Promoting independence
  • Providing support to develop and sustain social and community relationships
  • Developing skills to self-manage social interactions and community participation
  • Facilitating ongoing learning, training and development
  • Encouraging open communication with families, stakeholders and circles of support.

Host Provider Model

Your Life, Your Choice is a partnership between families and Lifestyle Solutions. The Alliance is committed to partnering with young people with disability and their families, to create opportunities which enable them to pursue a lifestyle that is personally meaningful, relevant and typically intertwined in the life of the broader community.

If you are a young adult or a student leaving school and have a disability which results in significant support needs, you and your family might be interested in a new form of post-school arrangements. You will need to be eligible, or potentially eligible, for the Community Participation Program.

The organisation will partner with people with disability and their families to:

  • Work out what the person would like to do.
  • Identify individual interests, skills and needs.
  • Work out a plan of action.
  • Assist to identify the support needed.
  • If needed, help develop skills to recruit and train paid support staff.
  • Facilitate connections with local community agencies.
  • Help work out how the funding can be used to achieve the plan of action.

Your Life, Your Choice, operates according to a set of principles, including the following:

  • Each person is a unique individual with talents, strengths, needs and personal aspirations.
  • People do best when they have decision-making and influence over how their life is lived, with whom, where and when. This creates supports which are relevant, responsive, flexible and creative.
  • The people closest to individuals with disability are often best placed to assist them achieve a full and meaningful life.
  • People, irrespective of the level or nature of their disability, generally do better when they have opportunities to build relationships with members of the broader community.

Your Life, Your Choice, will be of interest to you and your family if you would like to:

  • Create supports that are personalised and inclusive.
  • Actively participate in planning and guiding your own support.
  • Self-manage the Community Participation funding for which you are eligible.

Fee Structure

The cost of service is subject to the level of involvement that a person wishes to engage Lifestyle Solutions, the type of service purchased and the number of components purchased. An estimate of cost will be provided to the potential service users prior to engagement.
