All people aged 0 – 65 living with intellectual and/or mental health disabilities.

Organisational Information

Lutheran Community Care, Your Life Your Choice, Self-Directed Support Service is a host provider for individuals and/or their families offering:

  • Client engagement and liaison
  • Case management (including development of future goal plans and assistance with identifying and accessing services – brokerage)
  • Administration
  • Financial/budget management (includes reporting and acquitting to clients)

Our case management approach is focused on the individual’s and their family’s desires and goals.

We support individuals to live in ways that are as self-directed and independent as possible. We do this through assisting people to choose support services and explore community access options.

How our service works

The first step is to meet with an experienced case manager who will discuss the individual’s and/or family’s desires and aspirations. A working relationship will be developed where the case manager will support people to identify how they can achieve their goals. After the initial contact, case management is flexible and can be done face to face and/or through electronic contact.


Transparency is a vital part of our relationship with our clients/families. Our clients/families receive a monthly budget statement, either electronically or by mail, which will clearly show how much money has been spent and how much remains. Clients/families will be welcome, if they choose, to contact us in order to monitor funding and take an active role in managing the money.

Fee Structure

Costs for each host arrangement and what this will purchase

Cost of each host arrangement will be dependent upon the service each client requires. We anticipate the initial engagement will require approximately three hours. Further case management sessions would be expected to be on average 90 minutes per session. Hourly rate: $46.00 to $55.00 depending upon the expertise required.

Administrative fees

The cost of monitoring client expenditure and reporting on their expenses would be negotiated with each individual client at the time of engagement. The cost would be dependent upon the complexity and quantity of our involvement. Administrative charge out cost will be up to 15% of funds administered.


Any additional services will be provided at a charge out rate of $46.00 to $55.00 per hour depending upon the expertise required.
