Organisational Information

Multicap provides high needs disability, individual, respite and family support.

Multicap provides services and support for people with disabilities in the following regions across Queensland: North Queensland, Central Queensland, North Coast, Brisbane, South West and South East regions.

Host Provider Model

Individuals will be active participants in the design and delivery of their disability support, and Multicap will provide support and services as requested by the individual to assist them achieve their unique Support Plan.

Individuals and their family/carer will be invited to meet with a dedicated Multicap Self Directed Support Service Coordinator to discuss, explore and document their aims and desires for a quality life (Support Plan). Where agreed, Multicap will facilitate a person-centred planning workshop with all relevant stakeholders invited by the individual to assist this process.

The individual will decide on the level of responsibility they wish to have in the delivery of their supports (Service Contract). Individuals may wish to only use Multicap as the holder of the grant funds, or ask Multicap to work with them to identify the range of necessary and reasonable services and support available that will meet the documented outcomes in their individual Support Plan.

Agreement will be reached on the regular reporting and feedback required from Multicap by the individual, taking into account the requirements of the State Government as the funding body.

Multicap staff will meet regularly with the individual and their family/carer to review the service during the year, and at the end of the agreed contract period. The meeting at the end of the contract period will report on the outcomes achieved and check satisfaction with the services provided; and an invitation will be extended for the individual to participate in the process again resulting in a new Service Contract.

Fee Structure

Multicap will negotiate a scaled administration fee depending on the level of responsibility the individual elects to have. The level of support required may range from Multicap only holding the funds for an individual, to Multicap coordinating the full range of supports and services required.

The maximum fee for Multicap to coordinate the full range of services and support on behalf of an individual for 2013 is 15% of the total individual grant allocated to the individual by the State Government for specialist disability services.

The fee will cover the administration costs incurred by Multicap as Host Provider associated with coordination of the agreed services (Service Contract). The fee structure takes into account the need for the maximum funds possible to be available for necessary and reasonable services and support required to deliver the outcomes of the Support Plan.

Multicap also provides opportunities for individuals to access day service and arts based programs, centre-based respite services, social support programs, supported accommodation, and supported employment activities.
