Nextt has been delivering innovative, flexible community based support services for over 10 years. Throughout Queensland, New South Wales and Victoria, Nextt delivers a comprehensive range of community support services, including personal, home care, respite and 24 hr support, case management/coordination of supports, specialised community access and social inclusion services for people with a disability and/or mental illness.

Host Provider Model

Nextt’s model for services for people with a disability, champions self-directed support to enable people with disability, their family and carers to have choice and control over the disability supports and services they purchase to achieve positive outcomes.
By using a combination of disability funding and other resources to plan and purchase supports and services, people with disability, their families and carers have become active participants in the design and delivery of how they will meet their disability support needs. Nextt's host provider model is able to accommodate all individuals, wherever they elect to sit on the spectrum of self-directed support. Nextt will work with individuals to create a personally tailored service package allowing as much control as they wish over their supports and their life. Changing service levels is easy and regardless of the service level chosen, the individual is free to purchase additional support at any time.

Fee Structure

Introductory Consultation

  • One hour free consultation
  • Explanation of Self Directed Support, host provider principles, Nextt's approach to management of services and funding
  • Explanation of service levels and fees
  • Explanation of potential for interactions with others through informtion sessions, newsletter and community connections
  • Discussion of service options based on personal circumstances and objectives

Financial set up/Establishment

  • $200 - one-off fee (for two hours max)
  • Comprehensive assessment
  • Develop Individual Support and Funding Plan based on service level required
  • Establish Host Provider agreement
  • Create individual account in CareLink+

Fully Outsourced Plan

  • Funding administration fee - 10% of total package
  • Up to 50 Support Hours including four plan reviews (quarterly)
  • Charged quarterly
  • Individualised budget plan
  • Sourcing other service providers and community resources
  • Payment of invoices for services accessed
  • Monthly budget update reports
  • Online access to real time financial information (pending)
  • YLYC Newsletter (pending)
  • Quarterly acquittals to the department
  • Information regarding employing your own support staff
  • Additional assistance can be purchased separately - see Option 3

Fully Self-directed Plan

  • Direct Payment by Individual - 5% of total package
  • Up to 25 Support Hours including four plan reviews (quarterly)
  • Charged quarterly
  • Funds deposited into your account monthly
  • Online access to real time financial information (pending)
  • YLYC Newsletter (pending)
  • Quarterly acquittals to the department
  • Information regarding employing own support staff (factsheet)
  • Information and templates for record keeping

For this option the individual controls and manages:

  • A budget plan
  • Payment of invoices or services access or salaries for staff hired
  • Records of expenditure
  • Monitoring the balance of funding
  • Providing expenditure records and invoices to Nextt for acquittal to the department
  • Additional assistance can be purchased separately - see Option 3

Bespoke Plan (Option 1 or 2 plus service fee charged at an hourly rate per schedule of fees)

  • Additional assistance is charged as per most current schedule of fees/hour
  • Individual chooses Option 1 or Option 2 plus additional assistance charged per hour
    This may include but is not limited to:
  • Additional plan reviews (initiated by individual or at suggestion of Nextt)
  • Recruitment advice or support
  • Staff induction or training
  • Staff supervision or management
  • Sourcing information regarding insurance
  • Administration support (if on Option 2)
  • Sourcing information regarding available services (if on Option 2)
  • Assistance with funding applications

Exit Fee

  • $200 one-off fee
  • Balance transfer
  • Information handover
  • Final budget report
    A minimum of four weeks notice is required to prepare handover for transfer.

Travel is $1.05/km. If this fee is required it will be discussed and negotiated with participant prior to charge.
