Statewide: People living in Townsville, Whitsunday Coast, North West Queensland, Brisbane Metro, Greater Brisbane, Logan, Moreton Bay, Ipswich, Scenic Rim, Toowoomba, Lockyer Valley, Southern Downs, Tablelands and Far North Queensland

Target group

All people with a disability between 0 – 65 years of age.

Organisational Information

SOLAS is a community managed organisation that supports people with disability to live a life of choice, meaning, purpose and citizenship. SOLAS believes that Your Life Your Choice will provide people with disability and their families with an opportunity to develop a plan to live a good life.

At SOLAS we understand, acknowledge and respect that the people who use our services are best placed to determine their needs. SOLAS values inclusion and as such, management and staff work with people in a manner that respects self-mastery. SOLAS recognises the powerful contribution that people with a disability bring to the planning and support process and therefore SOLAS engages Peer Support Workers.

Host Provider Model

SOLAS as the Host Provider will work closely with the person and their family (if appropriate) to assist the person to develop his/her Lifestyle Support Plan. A SOLAS Lifestyle Support Plan is driven by the person's goals and aspirations, which give the person choice and control over his/her support needs.

SOLAS will assist people to understand and articulate their responsibilities including:

  • Planning
  • Budgeting
  • Organising supports and services
  • Reporting
  • Employing workers (if relevant)

A SOLAS Lifestyle Support Consultant, who will work collaboratively with the person and his/her family to develop the Lifestyle Support Plan, which will identify the reasonable and necessary supports and resources required to meet the individual's goals.

The Lifestyle Support Consultant will also assist the person and family to successfully navigate the journey.

Fee Structure

Initial Lifestyle Support Consultation


Establishment Fee: Lifestyle Support Plan

$550 Plan Development

Administration Fee

8% of gross package

Ongoing Direct Support Fees

$58 per hour depending on your support needs

Additional Consultancy Fees

$60 per hour Human Resource Management (staff recruitment)

Costs for other support services or fee-for-service can be negotiated as required (eg. additional planning activities, weekend, public holidays, early morning and late night supports).
