Southern Cross Support Services provides specialist referral information to allow people with a disability to make informed decisions on how to make the best use of their funding.

Host Provider Model

Self Directed Supports (SDS) System

Central to the operation of self-directed support is the resource allocation system which relates the resources available to support needs. This involves:

  • identifying the resources available
  • understanding how they currently relate to support needs
  • designing a self-assessment questionnaire to identify different levels of support needs
  • allocating points to each level
  • relating points to the amount of budget to be made available.

The aim is to produce a clear and fair way of allocating the budget in a way that people with a disability and their families can understand. This process is known as the Self Directed Supports (SDS) System and it produces an individual allocation for each person based on the support they need to achieve essential outcomes.

These seven steps have been developed over many years of work by In Control and offers a simple narrative to developing self-directed support for an individual.

Fee Structure

The following scale or fees will apply:

  • Initial establishment cost for host arrangement: $500 (under $10,000 package), $750 (over $10,000 package)
  • Ongoing financial administration: 5% of funding package
  • Fee for service / case management: negotiated on a case by case basis
  • Assisting with recruiting and managing Staffing (individuals and families sourcing and managing own staff): 5 – 15% of funding package.
