St John's Community Care is a community-based organisation that was established in 1997, and is playing a major role in community care in Far North Queensland. Host Provider Model

St John's Community Care Ltd (SJCCL) will assist you and your family/carer to develop skills and knowledge to set yourself up to manage your self-directed funding to your best ability and in your best interest. SJCCL aims to achieve the goal of having you ready to manage yourself as independently as possible.

The SJCCL Host Provider model makes sure that you and your family/carer is able to exercise maximum choice and control about how you are supported, who supports you and when, and how funding that you receive is directed and managed. Your needs and opinions are central to decision making and SJCCL gives your family/carers the due recognition for their role, rights and responsibilities.

SJCCL respects your individual needs, aspirations and goals in your involvement in your self directed planning and service through discussions with you. SJCCL’s host provider service is based on the principle of independence, decision making and choice, individuality and social inclusion.

When you use this service you can choose to involve SJCCL in as little or as much as you determine to accept responsibility for the planning, budgeting and organisation of your support.

SJCCL’s ability to respond to a person’s changing needs ensures maximum flexibility and choice for your needs.

SJCCL has a 16 years history of not only working with people with a disability, their families and the community, but also with Government Departments around funding matters. SJCCL can assist you and your family/carer to ensure that the legal, financial, accountability and human resources issues relating to self-directed funding are addressed and provide information and education to build your and your family's/carer's capacity to increase the level of control they exercise in these areas.

Agreements with SJCCL to facilitate services in the role of Host Provider are individualised and in accordance with your's and your family's/carer's needs.

Fee Structure

Initial information session:

  • free

Set up:

  • $300 one off fee

Full responsibility:

  • If the person accepts full responsibility for the planning, budgeting and organising their support, and funding is advanced to the persons bank account, SJCCL offers Host Provider model 1 in which the service user pays an hourly fee of $60.- for services provided as needed on an infrequent and irregular basis.

Most responsibility:

  • If the person accepts most of the responsibilities for the planning and organisation of their support and the remainder is purchased from SJCCL Host Provider, while funding may be advanced, reimbursed or held by the service provider, SJCCL will charge the service user 5% of their annual funding as fee.

Some responsibility:

  • If the person accept some of the responsibilities for planning, budgeting and organising their support and additional support in these areas is purchased; ie: most of the work is done by SJCCL, SJCCL Host Provider will charge the service user 8% of their annual funding as fee.
