Organisational Information

Suncare Community Services Inc. has successfully provided community support in Queensland for the past forty years.

Suncare delivers a range of flexible responsive services that meet individual needs with a commitment to supporting people who are aged, those experiencing a disability or mental illness and their carers and families who support them. These services are aimed to assist people to remain living independently at home and engaged within their local community. All services have a person centred approach acknowledging individual resilience and self-management and are culturally sensitive.

Suncare works in partnership with individuals, their families and their support networks. Through the provision of effective information and by developing mutual understanding and awareness; together identifying the needs and interests of the individual, supporting their choice and decision making in the formulation of their service plans with the tailored services to be delivered, where and when these are required by the person.

Host Provider Model

Suncare Active Choice Services will support clients who choose Suncare as their host provider to be supported through a process of consultation and planning. We aim to work with individuals to build capacity and to be informed of choices. Gaining awareness of their needs and interests Suncare can support their desired lifestyle and use these goals to formulate the individual service plan.

The plan will consist of –

  • Personal statements of desired outcomes and future directions, derived from working with the individual and their support network to brainstorm through wishes and aspirations and identify requirements and availability of resources, developing the persons capacity and skills to choose effectively and make informed decisions.
  • Identification of specific needs and service/resource requirements, to specifically define and list needs and interests, lifestyle choices, to define service responses and resources available, to identify support network opportunities within local community and peer connections, to establish responsibilities within choices, to establish priorities.
  • Establish a time line, a task schedule and responsibility assignment, to document a detailed plan of action and tasks required to achieve the choices made and the delegation and identification of who is responsible and when actions are expected to be achieved by defining the outcomes.
  • Strategic implementation and prioritisation plan, to establish short, medium and long term priorities, schedule review timelines and responsibilities, develop communication strategies and reporting requirements, define monitoring and access to information responsibilities.

Fee Structure

Suncare Active Choice Services will have a 10% administration fee; based on the individual funding package. The fee will be incorporated within the monthly accounting statements as an invoice and deduction.

These fees will be negotiable due to extenuating circumstances such as the capacity of the individual package and the extent of service and support required. This will be discussed and agreed prior to any change or adjustment to fee rate.

Initial discussion and assessment will formulate the details of the agreement to these rates and any other conditions required by all stakeholders.
