This service will be available to people throughout the whole of Queensland & there will be a variety of opportunities to communicate to best suit each person’s needs and wishes. Communication can occur:

  • Face to face and in person at SWARA’s premises at 101 Park Road, Woolloongabba
  • Face to face and remotely by using Skype or Face Time
  • Via email
  • By telephone
  • By writing
  • By text message
  • Any other mode of communication that is agreed upon by the person and SWARA

Target group

This service will be available to adults with a disability aged 18-65.

Organisational Information

SWARA is a community organisation that provides learning and life skills support to adults with intellectual and neurological disabilities. SWARA is a weekday service open 8.30am to 2.30pm. Based at 101 Park Road, Woolloongabba SWARA is easily accessible by public transport.

Host Provider Model

The model will focus on two main areas of delivery and actively promote personal choice and control in all areas: The two areas that will cover all life domains are:

  • Planning: SWARA will provide support for individuals to create & maintain & adapt a self directed plan focusing on goals and outcomes as identified by the person with a disability and their family/support people. SWARA will also administrate the plan on the individual’s behalf if that is their wish.
  • Hosting: SWARA will host individual’s funds in a special account and purchase services, support and equipment as directed by the person with a disability and their family/support people in line with their identified goals and program guidelines while providing information regarding expenditure status and balances.

Fee Structure

First meeting establishment fee of $150. This fee covers the cost of initial program planning, informal assessment and associated administration.

Support plans prepared at a cost of $50 an hour.

Any further meetings or agreed support concerning plans, preparation and review at $50 an hour.

Ongoing administration charge 10% of funding package. Administration support includes:

  • Regular statements of expenditure
  • Regular monitoring and reporting of all purchased items
  • Auspice of funding package
