Organisational Information

Breakaway Inc. is a community organisation that was established in 1992. Our aim is to provide a range of supports to people with disabilities and their families, in the Moreton Bay region.
Breakaway services include: centre based overnight respite, in home support, community access, accommodation support services, post school lifestyle programs, brokerage arrangements and emergency and crisis support.

Host Provider Model

Breakaway respects that you are the experts and know your needs. Self-direction is being the driver of your own life, your goals and how you reach them.

Self-directing your support means you, alongside with Breakaway, work collaboratively to design and manage your supports and services in the way that best suits you.

Breakaway will help you to explore the full range of options available.

Breakaway offer a range of services including:

  • development of individualised plans, and regular reviews
  • linking you to community resources, services and supports which are in line with your assessed needs
  • management of funds, and purchase reporting requirements
  • ensure services are provided in a culturally appropriate manner
  • supporting you with staffing
  • Individualised Lifestyle Facilitators to work alongside you in making your plan a reality
  • conflict of interest management
  • information about the YLYC - Use of Restrictive Practices Policy

Fee Structure

lnitial consultation - free

The initial consultation is free up to 2 hours.

Establishment fee - $800

This is a one-off payment which covers developing a plan for each individual, and a contract agreement. Information relating to what services match your needs will also be provided and/or sourced if required.

Ongoing fees

Weekly administration fee: $10
Minimum monthly coordination fee: $240 (4hrs per month at $60 per hour)

Ongoing fees include the following services:

  • ongoing planning and review
  • coordination of information and services
  • financial brokerage, monitoring and reporting

Additional costs which may be incurred (and outsourced if required):

  • staff recruitment
  • behavioural support
  • learning and development for individuals, families and/or support workers
  • resources to support the individual to manage their funds or any aspect of their plan

In instances of additional costs, these will be discussed with the individual and quotes provided prior to any arrangements being made.
