Organisational Information

Ability Care support individuals with a disability and their families to strengthen Your life, Your choice within self-directed support, who live in the geographical boundaries serviced by Ability Care, across Queensland. Ability Care provides disability services to people with all disabilities from 0 to 65 years.

Host Provider Model

Ability Care is an approved service provider through disability services and work within a person-centred framework offering assistance, support, and services in any way that is required by individuals with a disability and/or their families and carers. Transparent, innovative, flexible, responsive and personalised planning occurs, and how you receive services and utilise your funding is within your choice, control and decision making, to ensure that your vision for your life is achieved.

Ability Care will work with you to provide an array of services to ensure that your funding and resources are purchased and selected that best suit your needs. Such as:

  • Aids and appliances
  • Home and vehicle modifications
  • Personal care
  • Community access
  • Respite (in-home, centre based, community settings, host family and recreational/holiday)
  • Specialist accommodation support encompassing support for people to carry out essential activities of daily living
  • Challenging behaviour
  • Transitional accommodation
  • Domestic Assistance
  • Transport Assistance
  • Mobility and orientation training
  • Transition to work programs (volunteer work experience)
  • Therapies such as occupational, speech and physiotherapy, counselling and specialist behaviour support
  • Crisis and Emergency Support
  • Whole of life planning
  • Financial services, budgeting around daily living
  • Human Resource activities offered to individuals and/or families (ie, advertising for staff, recruitment of staff, training, professional development, discipline, supervision)
  • Case management and coordination
  • Local area coordination
  • Brokerage individual funding and other service providers
  • Administration support

Ability Care offer all service types outlined for direct purchase.

Ability Care promote independence and autonomy and will assist you in building informal networks, linking you with the community, and strengthening your skill and capacity to actively participate in your personal life with value and dignity, and a stronger voice.

Fee Structure

Establishment

An Initial Establishment Fee of $1,000 is applicable and includes the following:

  • Meetings with individual and families
  • Development of Host Agreements
  • Individual planning - person centred plan
  • Establishing finance systems
  • Tools for Self-Management
  • Information on employer responsibilities, legislative requirements

Model 1 - 6.5% of the package

Services provided by Ability Care:

  • Initial Assessment
  • Collaboratively design a self-directed action/case plan to incorporate needs, goals and desired outcomes
  • Act as administrator to transfer the funds to client’s account on a monthly/quarterly basis as agreed within the action/case plan.
  • Ability Care will be responsible for financial and Departmental acquittals.

Client and Family Responsibilities:

  • Client/Family will be responsible to source and action all supports and services.
  • Client/Family will be responsible to manage their expenditure in accordance to the action/case plan and provide information to Ability Care for financial acquittals.

Model 2- 10% of the package

Services provided by Ability Care:

  • Initial Assessment
  • Collaboratively design a self-directed action/case plan to incorporate needs, goals and desired outcomes
  • Ability Care acts as administrator and holds funds to make all payments (support worker and other brokerage services) at the direction of the client/family in accordance with the action plan.
  • Ability Care will be responsible for financial and Departmental acquittals.

Client and Family Responsibilities:

  • Client/Family will be responsible to source and action all supports and services.
  • Client/Family will be responsible to ensure all invoices for costs incurred are forwarded to Ability Care to action.
  • Client/Family will be responsible to manage their expenditure in accordance to the action/case plan and provide information to Ability Care for financial and Departmental acquittals.

Model 3 – 5% of package

In addition to Model 2, Ability will assist with:

  • Staff Recruitment, management, ongoing training and professional development on your behalf.
  • Brokerage Services – finding the right services, support and equipment for you.

Fee for Service – $85 per hour

Client/family may seek additional services through fee for service. Services offered include but not limit to:

  • HR and IR Assistance
  • Finance
  • Marketing
  • IT Support.
