Central Family Support’s mission is to deliver high quality and innovative support services for people with disabilities and their families/carers that promote choice, independence and life opportunities in an open and inclusive community.

Based in Emerald, Central Family Support has provided a variety of services across the Central Highlands and Western Qld region since 2000. Specifically, we offer:

Host Provider Model

Central Family Support prides itself on working closely with people to create individualised and flexible options that ensure personal choice and control in the way that a person receives services.
Central Family Support will work with a person, their family or chosen advocate to negotiate a host agreement which clearly outlines the level of involvement, roles and responsibilities a person wishes to play in the administration of individualised funding.

Central Family Support offers:

Level 1: Fully Self Directed Model

The person, family member and chosen representative will assume the full responsibility for all aspects of supports. The Central Family Support Host provider is responsible for hosting the individualised funding. Central Family Support Host provider will clearly outline to the person the costs associated with hosting the funding. The Host provider will ensure the agreement provides sufficient information and safeguards to conform to the requirements of the DSA 2006. Develop and manage your plan for support and funding.

Level 2: Partially managed and directed by the individual

Central Family Support Host provider will enter into a negotiable agreement with you which will identify the roles and responsibilities of each party. The schedule of fees will be identified relating to the level of Host Provider support required by you. In this model, the individual directs what services they require and assumes responsibility of specific roles as outlined in the contract between the individual and Central Family Support. The Host provider will ensure the agreement provides sufficient information and safeguards to conform to the requirements of the DSA 2006. The roles and responsibility of the individual and the host provider will be custom tailored according to what level of involvement and responsibility the person would like to assume in directing their funding.

Level 3: Fully managed and directed

The person assumes some responsibility in developing and directing their plan for support and funding. CFS will work with the person to develop a plan that meets their very unique and individual needs. In this model, the individual directs what services they require but assumes no administrative or financial responsibility. The Host provider will ensure the agreement provides sufficient information and safeguards to conform to the requirements of the DSA 2006.

Fee Structure

Level 1 fee structure

  • 5% of total annual funding package in the first year.
  • Establishment Fee of $600. This fee will be waivered for individuals who have a current plan with Central Family Support.
    (Fees and charges will be reviewed at the commencement of each financial year)

Level 2 fee structure

  • 5% to 10% of total annual funding package in the first year. Negotiated upon establishment of roles and responsibility of the Host provider.
  • Establishment Fee of $600. This fee will be waivered for individuals who have a current plan with Central Family Support.
  • Additional Case Management and Plan Review Fees of $55 per hour.
    (Fees and charges will be reviewed at the commencement of each financial year)

Level 3 fee structure

  • 10% of total annual funding package in the first year.
  • Establishment Fee of $600. This fee will be waivered for individuals who have a current plan with Central Family Support.
  • Case Management and Plan Review Fees of $55 per hour.
    (Fees and charges will be reviewed at the commencement of each financial year
