At Community Lifestyle Support Inc. our vision is to empower people with disabilities and our mission is to be a person centred service within the Bundaberg community that equally includes people with disabilities in all aspects of community life.

Host Provider Model

Community Lifestyle Support (CLS) ‘Empowered to Choose’ (EtC) Self Directed Funding Host Provider model is based on empowering people with a disability and their families to choose and control the life they want, achieving the goals they want, working towards the future they want.

To achieve these outcomes requires collaboration and flexibility; ensuring we are all working together.

The ethos of self-direction is embodied in the model from first contact and throughout - with the person with a disability and their family deciding how much assistance/support will be required to meet their needs/goals, and support their plan to become a reality.

The EtC model of self-directed host provider recognises that capacity/willingness to make decisions and direct funding can change over time and the circumstances of a person’s life and the level of assistance by EtC to manage self-directed funding will change in accordance with that capacity and/or willingness.

Good planning is the key to successful self-directed support. The EtC Self-directed funding model recognises the uniqueness of each person and family and supports planning that is suitable for that person and their family. The plan may be an informal process or a more formal process assisted by EtC (utilising PATH. – Planning Alternative Tomorrows with Hope), the end result will be a person centred plan with that will be used to direct self-directed support, and assist reviewing and updating at agreed times.

Fee Structure

Option 1

The person accepts full responsibility for the planning, budgeting and organising their support. Funding is advanced to the person’s bank account:

  • Setup Fee for this level of support will be $500, this includes setting up Care-Link cost centre for client, reviewing and agreeing to the person’s prepared plan, establishing relationship between EtC, person and their family and CLS Financial Department.
  • Ongoing reviews and updates to person’s plan will be $80 per hour.
  • Case Management if required to ensure flexible and responsive supports will be $80 per hour.
  • Advancing funding to person’s bank account 1% Administration fee.

Option 2

The person accepts most of the responsibilities for the planning and organising of their support. The remainder is purchased. Funding may be advanced, reimbursed or held by the service provider –

  • Setup Fee for this level of support will be $500; this includes setting up Care-Link cost centre for client, reviewing and agreeing to the person’s prepared plan, establishing relationship between EtC, person and their family and CLS Financial Department and other relevant areas in CLS.
  • Planning utilising PATH. (Planning Alternative Tomorrows with Hope) will be $80 per hour.
  • Review and updates to person’s plan will be $80 per hour.
  • Case Management including but not limited to: ensure supports are flexible and responsive to changing needs, organising purchase of supports/resources/brokerage will be $80 per hour.
  • Managing funding – including but not limited to: paying invoices, reimbursements, advances, brokerage, assistance with budgeting if required – 3% administration fee

Option 3

The person accepts some of the responsibilities of planning, budgeting and organising their support. Additional support in these areas is purchased:

  • Setup Fee for this level of support will be $500, this includes setting up Care-Link cost centre for client, reviewing and agreeing to the person’s prepared plan, establishing relationship between EtC, person and their family and CLS Financial Department and other relevant areas in CLS.
  • Planning utilising PATH. will be $80 per hour.
  • Review and updates to person’s plan will be $80 per hour.
  • Case Management including but not limited to: ensuring supports are flexible and responsive to changing needs, assisting in establishing peer supports, networks and community connections, organising purchase of supports/resources/organising employment of person’s workers will be $80 per hour.
  • Managing funding – including but not limited to: budgeting funds, paying invoices, reimbursements, advances, brokerage, paying wages to workers – 5% administration fee.
