Organisational Information

Co-Ordinated Support Services (CSS) provides support to Children and Young People and People with Disabilities within Central Queensland.

We pride ourselves on responding to the care needs on individuals in a therapeutic way that supports your needs, working closely with you, and others of your choice to explore, refine and document goals, needs and aspirations.

Host Provider Model

As a host provider, Co-Ordinated Support Services (CSS) will play an integral role in assisting people with a disability and their families, wishing to self-direct their funds, to take greater control over the supports and services they receive.

CSS’s model of support is designed around the needs, goals and aspirations of those accessing our service. CSS will engage with the person with a disability their partner, family, friends, community, and any appointed decision makers. Through good planning processes and with a careful understanding of the person, we will support the person to explore, articulate and document their personal goals, needs and aspirations. These goals may be identified as short term (less than 1 year), the medium term, or longer term goals (five years, plus).

We will assist the person to identify:

  • what supports are available based on their needs
  • identify and consider the resources available to them
  • discuss and identify the priority/s for use of available resources
  • ensure this information is documented this in their individual plan

Once a plan is developed and agreed, we will work with the individual to select the support staff of their choice from an organisation of their choice, this could be another community organisation or CSS.

Each person’s plan will be monitored and adjusted as required but no less than six monthly.

CSS will take a systematic approach to evaluate service delivery. We will request regular feedback from individuals accessing our service, in addition to undertaking external

feedback from stakeholders and the community. External feedback will be sort through a credible organisation independent of CSS every 3 years.

Fee Structure

Step 1.

An Initial Interview of approximately 1 hour will be used to assist us to assess if we have the capacity and contacts to meet individual requests. $0 cost to the client.

Step 2.

Assistance and support for the Client and their families to define their needs and select services from organisations and individuals they wish to have deliver the required services. Estimated meeting time of 4 – 5 hours at $60 per hour.

Step 3.

On acceptance of the services CSS are able to offer:

  • Administrative costs will be between 7% and 10% of the funding package available, to be negotiated as a component of the service agreement.

Indicative Costs for staff participation in host provider services are $47 - $60 per hour of services purchased including on costs and overheads.
