All people with a disability aged 0–65, including Aboriginal and Torres Strait Islander peoples and those from diverse cultural backgrounds.

Organisational Information

For more than 60 years, Cootharinga has supported people with disability living right across North Queensland. We support over 600 people with a range of person centred and flexible services, in their own homes, and in the community. We also offer centre based respite, and allied health services (physiotherapy, speech and occupational therapy). We offer the only Rehabilitation Technology Service in Australia that is based outside of a capital city. This service customises wheelchairs, mobility equipment and other aids to daily living. Our focus is to maximise independence and comfort.

Over the last two years, Cootharinga has developed some highly successful and recognised and awarded options for accommodation for people with disability. We have created several different housing projects that offer homes that are smarter, greener and fully accessible.

Host Provider Model

Our model is designed around the needs, goals and aspirations of the person we support. It generally involves the following series of steps (but this obviously can vary depending on the individual, their circumstances, and urgency of their support needs):

We engage with the person. As necessary and as directed by the person, we also will engage with their partner, family, friends, neighbours, community, and any appointed decision makers. Through good planning processes and with a careful understanding of the person, we support the person to explore, articulate and document their personal goals, needs and aspirations. These are generally considered in the short term (less than 1 year), the medium term, and the longer term (five years, plus).

The next step is to assist the person to identify the following:

  • what possible supports are available to meet their needs
  • consider the resources available to them
  • what do they identify as the priority for use of available resources
  • Document this in their individual plan

Once a plan is developed and agreed, we work with the individual to select the support staff of their choice from our organisation, or in fact from any other organisation. As well as support, any resources, or equipment are sourced and put in place.

Each plan is monitored regularly with the person, and adjusted as desired by them. Every plan is reviewed at least each six months or sooner if desired by the person.

Fee Structure

All potential service users will be offered a free consultation with our Your Life Your Choice Coordinator. Based on the support that will be sought, a detailed quotation will be supplied to you in writing An email can be sent on the spot; while printed copies will be posted within one working day of meeting with you.

During these discussions, our Coordinator and the individual (and others if requested) will discuss a range of options for their self directed support. Individuals and families can take on full responsibility, partial responsibility or just some responsibility.

Our charges are based on the actual effort and time involved in providing you with assistance. Our plan will include detail of the following:

  1. We typically spend between two and four hours with you to develop and implement an initial plan of support. Ongoing review is available as required by you. Our cost for this support, care coordination and case management is currently $58.70 per hour
  2. Any administration support requested from us ($41.20 per hour). This will depend on the level of responsibility you request we take on. Most packages will require one hour of admin support per month.
  3. Any brokered services will be at the cost of the external provider (plus 5% to us if we process and pay the brokers, if requested). No surcharge applies to services from Cootharinga.
  4. Any costs or fees to be covered by the individual (e.g. cancellation fees, services and equipment not covered by DS funding)

Note: Our charges can be reviewed and we will give you a minimum notice of one month if a change is to be made. In cases of genuine financial hardship, we may have access to additional resources or funding to offset some of the costs of supports engaged.
