Organisational Information

CQ Community & In-home Care is a well-established community support provider serving the community of Mackay for the past four years. We achieved recurrently funded service provider status with Department of Communities in 2010 upon providing support for people with disabilities. Since this time we have provided support for many people providing including flexible in-home support, respite services and community engagement. The host provider model of care will be an extension of this, utilising a range of models from traditional service provision through brokered arrangements or utilising our service through to client self-management. All parts of the self-direction spectrum will be included in our model, heavily focussed on the service user and providing the best fit of services to enable them to live their life as they choose.

Host Provider Model

CQ Community & In-home Care operates from a flexible framework that understands and appreciates the difference and uniqueness of each person and their family and utilises an individualised and collaborative approach to planning care. We pride ourselves on individualised support of service users, and have done over the past four years and involve the client and their family members in all changes and updates to our Individual Lifestyle Plan. We understand the flexibility in providing support to clients as their support needs and lifestyle decisions change; moving from school to post school into the workforce according to their abilities and goals. We do this through initially meeting with the client and their family where we discuss their needs and their current levels of support, both informal and formal and their short, medium and long term goals. The self-managed model will enable the person or their family to decide on supports required to benefit them and ensure the best outcome for the person. The clients can change or modify their support requirements if they begin to feel overwhelmed as the manager of their own support through all models of care and we would envisage that someone who takes some
responsibility may decide to take full responsibility for their care as they feel more comfortable with the new skills they have developed.

Fee Structure

Initial interview

  • (up to 2 hours) free at CQ Nurse office

Establishment fee

  • Model 1 & 2 - $700
  • Model 3 (includes initial assessment) - $890

Model 1

If you choose full responsibility for managing your lifestyle and disability support. Within this model of Host Provision, CQ Community & In-home Care will work with you and your family ensuring Disability Services reporting obligations are met. Following initial consultation with our coordinator you and/or your family will have control over planning your budget, the supports you use including your own staff and budgeting and establishing financial control to support you. You and/or your family will have access to our Coordinator who will work with you, under your control, to facilitate client centred support options for you. Administration cost 9% of funding package invoiced each quarter

Model 2

If you choose to be mostly responsible for managing your lifestyle and disability support. CQ Community & In-home Care will work with you and/or your family to assist you to manage some aspects of your funding, and those you are not able to manage will be coordinated by our service. You can choose aspects of support you feel comfortable managing, and which aspects you want us to assist you with. We will ensure Disability Services reporting obligations are met. You and your support network will be able to work directly with our locally based Coordinator to assist you to choose the best options for you. This will include the design and follow through of the Individual Support Plan. 10% of funding package invoiced each quarter.

Model 3

If you choose to be only partially responsible for managing your lifestyle and disability supports. Included in this model, CQ Community & In-home Care will manage the funding for you. We will consult with you and your family to establish an individualised support plan based on the needs you identify in the assessment process. This will enable us to:

  • establish community engagement plans and individualise your support and lifestyle options;
  • monitor your usage of available funding;
  • disburse funding to service providers involved in providing direct support to you;
  • ensure Disability Services reporting obligations are met; and
  • ensure other established links with community providers are utilised in building a support package integrating the person into the community around employment and utilising their abilities.

12% of funding package invoiced each quarter

$85.00 per hour, including recruitment services

Exit fee

We require 4 weeks notice to exit the service/transfer provider. Administration fee of $250 applies.
1 week's notice is required to change model, with no fee attached.
