All people with disability aged 0-65 years.

Organisational Information

Douglas Shire Community Services Association Ltd (DSCSA Ltd) is a community based organisation with a 13 year history in the Douglas Shire supporting children and adults who have disability and their families and support networks.

Our vision is to enable eligible individuals and their families greater access to a range of supports and services they need as they move through the different stages of their life.

Our services have a strong focus on empowering individuals and building on their strengths by:

  • encouraging and supporting decision making and choice
  • developing an individuals skills and abilities
  • adapting to meet an individuals changing needs
  • developing resilience and independence
  • promoting and supporting inclusion in community and positive relationships

We work collaboratively with our service users, their support networks and the community to facilitate a range of options and opportunities which promote active participation for all. We value and respect diversity and difference and have the capacity to deliver culturally appropriate services.

At DSCSA Ltd Community Connect we encourage people to be involved in planning and creating the supports they choose to live life as a valued and included member of the community.

As a host provider DSCSA Ltd will play a pivotal role in supporting people with disability and their families or carer to take greater control of the types of support they receive and who provides that support.

Host Provider Model

As a host provider, DSCSA Ltd Community Connect will play an integral role in assisting people with disability and their families to take greater control over the supports and services they receive.

The DSCSA Ltd Mossman Community Connect model of service provision is based around a client centric approach where individual supports can be selected based upon the need and cost to the individual. DSCSA's services include:

  • support plan development
  • long term needs planning
  • service provision or service coordination support
  • accounting, auditing and financial acquittal

DSCSA Ltd Mossman Community Connect will engage with the person with a disability their partner, family, friends, community, and any appointed decision makers. Through good planning processes and with a careful understanding of the person, we will support the person to:

  • identify what supports are available based on their needs
  • identify and consider the resources available to them (formal and informal)
  • discuss and identify the priority/s for use of available resources
  • ensure best value for money

Once a plan is developed and agreed, we will work with the individual to select the support staff of their choice from an organisation of their choice. Change between options is available, with support provided to develop skills to become more empowered with taking on more responsibility as confidence grows ensuring each person has the option to mix and match the type and level of assistance they require, to turn their package into the support they need.

Each person's plan will be flexible and will be adjusted based upon the changing capacity, needs and wants of the individual. DSCSA Ltd Community Connect will take a systematic approach to evaluate service delivery. We will request regular feedback from individual's accessing our service, in addition to undertaking external feedback from stakeholders and the community.

DSCSA Ltd Community Connect is able to deliver these planning services in a number of ways including face to face, telephone, web/email based. We are a local Mossman based organisation outreaching to the Cooktown and Cairns region and will ensure that there is one point of contact for you in your communications with us.

Fee Structure

A free initial one hour consultation will be provided to discuss what DSCSA Ltd Community Connect.

Establishment fee

A one-off initial establishment fee of $600 includes:

  • development of a service agreement
  • definition of the roles and responsibilities of consumers and DSCSA Ltd Community Connect
  • interpretation of support needs to ensure supports purchased are consistent with the approved items on the consumers plan and within the funding guidelines

Full Self Direction

Administration fee of 7.5% of the individual total funds applies to this model. The remaining of the total funding will be placed with the individual, in a separate bank account, with monies being transferred from DSCSA Ltd Community Connect on a monthly basis. The administration fee provides support facilitators to assist with the establishment and development of self-directed support arrangements. The fee covers:

  • preparation and provision of quarterly fund expenditure reports to the person responsible for coordination and ensures the accountability of financial and service obligations required by the department
  • training and mentoring
  • annual review of support plan
    Work undertaken by DSCSA Ltd Community Connect over and above that outlined in the agreement is charged at a flat rate of $50 per hour.

Moderate Self Direction

Administration fee of 9.5% of the individual total funds applies to this model. This fee provides for case facilitiation and includes support with:

  • preparation and provision of quarterly fund expenditure reports to the person responsible for coordination and ensures the accountability of financial and service obligations required by the department
  • payroll (on invoice only via use of subcontractors)
  • tracking of support hours and outputs
  • payment on invoice of other services (brokerage fees, allied health professionals, aids and equipment)
  • management of unspent funding reserves on behalf of the person receiving support for future expenses within funding guidelines
    Work undertaken by DSCSA Ltd Community Connect over and above that outlined in the agreement is charged at a flat rate of $50 per hour.

Some Self Direction

Administration fee of 12% of the individual total funds applies to this model. This fee provides for case facilitiation and includes support with:

  • preparation and provision of quarterly fund expenditure reports to the person responsible for coordination and ensures the accountability of financial and service obligations required by the department
  • payroll (on invoice only via use of subcontractors)
  • tracking of support hours and outputs
  • payment on invoice of other services (brokerage fees, allied health professionals, aids and equipment)
  • management of unspent funding reserves on behalf of the person receiving support for future expenses within funding guidelines
  • support with human resources (staff - advertising, recruitment, induction, training, professional development, supervision, performance management etc.)
  • insurances (public liability, professional indemnity and WorkCover)
  • full financial responsibility and accountability for expenditure of funds
    Work undertaken by DSCSA Ltd Community Connect over and above that outlined in the agreement is charged at a flat rate of $50 per hour.

Consumers can move between models with renegotiation of fees ie. consumers may start off with needing a lot of support but over time they take on more tasks, reducing the amount of support required.

Fees and charges will be reviewed on a regular basis.

Please note DSCSA Ltd may negotiate an increase in administration fees with you should the need arise.
