Excelcare/Your Choice - Our Priority (Previously Keppel Community Care)

Children and Adults with a disability, including Aboriginal and Torres Strait Islander peoples and those from culturally diverse backgrounds.

Organisational Information

For 19 years Excelcare has supported people with a disability, people with a mental illness, frail aged people and children in care. Your Life Your Choice program (Your Life – Our Priority) is a Person-centred planning model that is designed specifically to 'empower' people, to directly support your goals, ambitions and social inclusion. Your Choice – Our Priority is a process that focuses on discovering your gifts, skills and capacities, and on listening for what is really important to you. It is based on the values of human rights, independence, choice and social inclusion, and can be designed to enable you to direct your own services and supports, in a personalised way. Excelcare continues to provide direct support to many people who live in our community.

Host Provider Model

Our model is designed to ensure that you are central to the design of your service, which you can then choose to manage in a variety of ways to suit your skills and abilities.

Excelcare will work with you and your family, to ensure all reporting obligations to the funding department are met, however you or your family will have the maximum amount of control over planning your budget, the supports you use (including staff), and the financing of those supports. You and/or your family choose which elements you feel comfortable with managing, and which elements you want Excelcare to assist you with. You and/or your family may choose to have access to a locally based Individual Support Consultant who will work directly with you, under your control, to facilitate person centred options for you.

This may include:

  • creating a person centred support plan that identifies individual supports and lifestyle options;
  • monitoring that the plan is on track or facilitating adjustments in collaboration with you and/or your family;
  • managing all administrative and reporting requirements (budgeting, invoicing, payments, reporting to Govt. etc); and
  • using asset based community development techniques, supporting you to maximise your natural support assets within your community.

Our model will be unique in that we will adapt our host model according to your capacity and desire, we can support you to transition with different levels of funding management, according to your needs and choices.

Fee Structure

Fees are subject to negotiation depending on the complexity of support needs and the degree of self management that you will undertake. A detailed budgeting tool is available. It will be completed in conjunction with you in order to provide a comprehensive guide to the cost of your service, including direct support hours, clinical support, case management, assessment, sub-contracted services and other purchases from your individual funding.

Establishment fee

One-off establishment fee $500

Administration charge - 7.5% - 15%

Charge depends on level of management required.

This includes:

  • Develop a Service Agreement (host agreement) with you (Between client and Your Choice Our Priority)
  • Sign off on funding agreements with Disability Services
  • Pay bills or monthly reimbursements to your account.
  • Keep accurate and separate accounting records of funds received and expended
  • Provide monthly financial reports
  • Department Annual financial returns
  • Submit NMDS reports
  • Hold a contingency amount on your behalf for future expenses as requested by you (subject to DS guidelines)
  • Formal endorsement of your Self-Directed Support Plan

  • Initial setup
  • Helping you identify individual supports and lifestyle options
  • Helping you develop your lifestyle support plan including behaviour support plans where these are needed
  • Assistance managing invoices/evidence of supports
  • Ongoing case management
  • Assisting you with your recruitment of staff
  • Training your staff
  • Management of your payroll
  • Sourcing and negotiating services for you
  • Provision of a range direct support services
