Access Recreation's focus is on building self-confidence, independence, and opportunity to exercise personal choice.

Host Provider Model

Access Recreation (AR) Host Provider Services is a flexible service that is based on you identifying how you want to reach your goals and dreams and how you choose to manage and meet your disability support needs with your funding package. We believe that the 'wisdom resides in each of us' and people are experts in themselves. We will work with you to develop flexible and creative approaches to meet your desired lifestyle and disability support needs, all within the Your Life Your Choice guidelines and framework. Our model of support recognises the importance of meaningful mainstream and community involvement and draws upon our extensive knowledge of the local community.

In our model, a locally based coordinator will be the key point of contact. The coordinator will work individually with you and/or your family so you become an active participant in the design and delivery of your disability support. You will have a level of control that you are comfortable with to plan, purchase and select supports and services that meet your individual needs.

Our coordinators are experienced in person-centred planning and our coordinators will work closely with you (and your family if applicable) to develop a plan which will guide the disability supports required, considering both formal and informal supports and mainstream opportunities. The coordinator will also work closely with you to determine your preferences around:

  • The level of control and involvement you want in managing your funds and supports. We can act on your behalf to organise some or all of your chosen supports and services (self-direction) or you can organise your own support services, including employing your own support workers directly (self-management).
  • Who provides the support you want. We have extensive local knowledge and will provide a catalogue of the range of services available, both formal and informal, mainstream and specialist options.
  • What the support looks like and what is important to you in how support is provided.
  • What level of service you want from us as the host provider. This may include managing all administrative and reporting requirements (budgeting, invoicing, payments, reporting etc.).

We understand that your needs and goals may change over time. AR Host Provider Services is committed to being flexible and responsive and we will support you to transition to different levels of funding management should you desire.

Fee Structure
