Your Life Your Choice Host Providers

Gladstone Community Linking Association (GCLA)

Available to

Rockhampton, Gladstone and Capricorn Coast.

Target group

All people with a disability aged 0-64 years and their carers, with no restrictions on culture, religious, level of care, behaviour, disability type or complexity of support requirements.

Organisational Information

GCLA's mission is to ensure people who have a disability are respected, valued and accepted as integral members of their community.

GCLA is a person centred service; power is given to the people who use the service. This philosophy dictates our organisational structure and our preparation for the NDIS.

GCLA has provided services for people who have a disability and their carers/families in the Gladstone region for the past 19 years. GCLA prides itself on providing a person centred service embracing the philosophy of Social Role Valorisation, with consistent quality best practice. This is reflected in audits, service referrals and people choice.

Host Provider Model

GCLA is always empowering and supporting individuals, carers and their families to take the lead with regards to their support needs whilst accepting that everyone is an individual and each person’s requirements will be different.

Rather than offer one rigid host provider model, GLCA offers a range of options suited an individual’s needs and their desired level of involvement in directing their care. This might include supporting individuals and their families to self-direct some, most or all of their care package. Our skilled, professional and experienced personnel will work in partnership with individuals and their families to find the right option for them.

To ensure that the on-going plan is still the best fit for the person, GCLA will review the support plan on a quarterly basis, as a minimum. However, we are always striving to ensure that the individual has the maximum choice and control about how they are supported.

Without exception, GCLA will report the outputs on behalf of the individual/carer/family and maintain and report the necessary financial data.

Fee Structure

Dependant on the required level of involvement, GCLA will charge an establishment fee ranging from $500 for existing clients to $1000 for new clients.

GCLA will charge a flat administration fee of 5% of the value of your funding package. Any additional case management support can be purchased as needed. A specifically tailored quotation will be provided for your approval before the commencement of any work, again ensuring that the individual can make an informed decision and retain choice and control over their support requirements.

Alternatively, you may choose to self-manage your own funds. This would be charged at a flat fee of 3% of the value of your funding package.

GCLA is responsible for reporting the outputs on behalf of the individual, carer and/or family, and maintaining and reporting the necessary financial data.
