Hinterland Services

Available to

Dysart and Moranbah (within the Isaac Regional Council Region)

Remote service delivery available to Tieri, Nebo, Middlemount, Clermont, Glenden and surrounding areas

Target group

All people with disability aged 0-65 years, and families of children with disability up to the age of 18, including individuals who identify as Aboriginal or Torres Strait Islander or people with culturally and linguistically diverse backgrounds.

Organisational Information

Hinterland Community Care Inc has been providing a flexible quality service in the Isaac Region since 1992. The organisation's services are based on the values of respect, individualism, inclusion, trust, transparency and commitment. Hinterland Community Care is committed to ensuring:

  • Individuals exercise choice in the supports they receive
  • All service is driven by the individual
  • The individual is an active participant in decisions that impact them

Hinterland Community Care is a strong advocate for community inclusion and believes in each person's right to be an individual. The organisation offers a range of individual and group specific programs that are developed in consultation with each individual. Staff members are professionals in their field who are police checked and have valid Blue Cards and Yellow Card exemption.

Host Provider Model

Hinterland Services provides individuals and families with both choice and flexibility. We work alongside each individual to ensure they can voice their goals and aspirations clearly and gain a strong understanding of the service options available to them. We support the individual to choose the model of service that best suits their current needs.

Hinterland Services recognises that the individual's needs may change over time and they can be assisted to move within or between models of service as required.

Fee Structure

Initial contact: Phone interview of up to 15 minutes free of charge.


A non-refundable once-off fee of $750 is applicable on establishment of package. This will include:

  • Initial meeting (face-to-face, skype or phone) with individual and family to design a self-directed individual plan incorporating needs, goals and outcomes
  • Establishing financial systems to track all expenses and wages
  • Establishing individual as a client within Hinterland Services and for Disability Services reporting
  • General administration costs (photocopying, printing, etc.)
    Further support may be provided at a fee per hour plus travel costs where necessary.

Platinum Package - Self directed - 6.5 % of package

The individual takes full responsibility for all aspects of their care plan including employment requirements, insurance, legislative compliance, purchasing and managing expenditure and providing necessary information to Hinterland Services for financial acquittals.

Hinterland Services will be responsible for all financial and departmental acquittals, transferring funds to individual's account and issuing quarterly financial reports and survey. Individuals will also be provided with:

  • handbook detailing employer responsibilities, legislative requirements etc.
  • one hour training session in the use of tracking system for expenses and wages

Diamond Package - Partially Supported - 8% of package

The individual takes some responsibility for some of the aspects of their care plan which may include employment of staff, human resources requirements, insurance, legislative requirements, purchasing and managing expenditure in accordance with their individual plan and providing necessary information to Hinterland Services for financial acquittals.

Hinterland Services will be responsible for some of the plan management and financial and departmental acquittals. Hinterland Services will transfer funds into client's account as agreed within the care plan and issuing quarterly reports.

Gold Package - Fully Supported - 10% of package

The individual takes limited responsibility but will actively participate in all aspects of planning and make choices based on their assessed needs and specific goals. They will forward all invoices to Hinterland Services for payment, and provide information to Hinterland Services for financial and departmental acquittals.

Hinterland Services will be responsible for plan management and financial and departmental acquittals. They will act as an administrator, hold all funds and make payments at the direction of the individual/family, in accordance with their individual plan. Hinterland Services will also manage human resource needs, including recruitment and staff monitoring.

Plan Review

All individual plans will be reviewed annually or as required at a cost of $170. This includes a one hour meeting to review the plan with the individual, development of revised plan and administration costs.

All fees are subject to change.

All plans and services will be designed and managed according to the Disability Services Act 2006.
