Brisbane Metro & Greater Brisbane, Ipswich and Scenic Rim, Logan, Moreton Bay and Gold Coast, Sunshine Coast and Somerset regions.

Target group

Children and young people aged 0 – 21 years.

Organisational Information

InFocus Disability Services is a Brisbane-based support organisation for families of children/young people aged 0-21 who have high support needs arising from a variety of disabilities including physical, sensory, intellectual and autistic spectrum disorders.

We aim to enhance capacity and empower families through provision of tailored support, information and choice. InFocus services are designed to meet the specific requirements of each child/young person and their family.

Host Provider Model

Our InFocus team is committed to genuine family centered support being realized through self-direction, choice and flexibility.

Families in receipt of an individual funding package (e.g. from Disability Services) and who have a child/young adult between the ages of 0 and 21 years with a disability may choose to contract InFocus to partner with them in the development and roll-out of supports as identified by each family.

When contracting with InFocus as a host provider, the family may choose to self -manage their individual funding package after initial establishment or seek the involvement of a Facilitator to the degree requested in planning, delivering and managing their supports.

The extent to which each family self-directs or self-manages is of their choosing and will be reflected in an individual InFocus contract. Renegotiation can occur at any time due to changing circumstances preserving flexibility and choice. Use of funds must be in accordance with the guidelines of the relevant funding body.

Fee Structure

An establishment fee (one-off) of $600


  • development of PATH Plan and short term personal support plan
  • development of host agreement
  • training for individuals/families in self-direction/self-management
  • introductory workshop series
  • establishing financial and accountability systems and training
  • self-direction handbook and tools for accountability

Host Provider Administration Costs (10% 0n total funding package)


  • auspice of funding package
  • financial support – grants management, direct payments, annual budgets, monthly financial statements
  • paying invoices as approved in support plan or services accessed
  • accountability and reporting responsibilities to Disability Services
  • administrative support as required

Facilitator Support $90 per hour (purchased as per level requested)


  • additional support for development of initial support Plan
  • review of support Plan and contract as required
  • assistance with referral and linking to services
  • negotiation with service providers and brokerage services
  • management of purchases of goods and services
  • assistance with staff recruitment and sourcing information regarding OHS, legislative requirements, insurances etc.
  • assistance with funding applications.
