Target group

Life Stream Australia provides support to children and adults with varying disabilities, specialising in supporting individuals with an Intellectual Disability. Life Stream Australia's Host Funding Program is open to any and all individuals currently in receipt of funding through the Your Life Your Choice Program.

Organisational Information

Life Stream Australia (LSA) is a state wide organisation providing unique and varied supports to individuals with disabilities. Life Stream Australia facilitates a wide range of opportunities across Queensland to enable people with disabilities to participate, develop skills and be active members of their communities. Life Stream Australia believes that through participation in community activities, recreation, leisure and sport, and having friendships and responsibilities, people develop all types of skills and lead healthy, active and meaningful lives. Life Stream Australia has considerable experience in the development of programs and delivery of supports to individuals with complex support needs. As a state wide organisation, Life Stream Australia is led by a Chief Executive Officer, under the direction of a Board of Directors, with localised offices in Brisbane, Rockhampton and Mackay. Through state wide networks, both formal and informal, Life Stream Australia has relationships established with services across Queensland.

Host Provider Model

Life Stream Australia, through consultation with key staff and prospective service users has established a flexible and individualised host funding support model. Life Stream Australia proposes a state wide model of service, delivered remotely with the capacity to engage in person at any of the current Life Stream Australia offices. Life Stream Australia will employ a state wide Hosting Coordinator, who will operate outside of all other aspects of Life Stream service delivery, engaging only with other Life Stream Australia services in the event individuals and families choose to utilise Life Stream Australia services. Details regarding the Host Funding Program will be made available on Life Stream Australia’s website for all potential service users.

Fee Structure

All users of the Life Stream Australia Host Funding Program will pay an establishment fee of $500. This fee is not negotiable and provides for the establishment of internal administrative protocols for each package.

Life Stream Australia has established a fee structure dependent upon the model of support chosen by each individual service user. There are three model options.

Model A – Supported

This model provides the greatest amount of support to individuals and families. (Max 10 hrs per month Hosting Coordination)
LSA will:

  • Develop, publish and review the Services and Support Plan.
  • Recruit staff (if applicable), negotiate with service providers, and/or broker services.
  • Manage all legal and industrial aspects of employment of staff.
  • Facilitate communication between the individual, the family and the service providers.
  • Facilitate group discussions and/or meetings with service agencies with the individual and their family.
  • Maintain regular contact (twice monthly) with the individual and/or their family to ensure service delivery is aligned with the Services and Support Plan.
  • Provide advocacy support to the individual and their family to ensure services and supports are meeting identified needs.
  • Ensure timely payment of services and support.
  • Provide financial accountability and security for the payment of funds to LSA on behalf of the family, and ensure appropriate accountability and acquittal.

The individual and their family will:

  • Provide input into the plan, sharing needs, wants and hopes for services and support.
  • With support, choose appropriate service provider options.
  • Participate in the selection and recruitment of staff where required.

Model B – Shared

This model provides the opportunity for increased involvement of the individual and family members in negotiating, developing and leading the supports requested. (Max 5 hrs per month Host Coordination)
LSA will:

  • Develop, publish and review the Services and Support Plan.
  • Recruit staff (if applicable), negotiate with service providers, and/or broker services.
  • Manage all legal and industrial aspects of employment of staff.
  • Participate in group discussions and/or meetings with service agencies with the individual and their family as requested.
  • Provide mentoring and support for individuals and their family to increase their skills in self-managed funding.
  • Maintain frequent contact (monthly) with the individual and their family regarding service delivery.
  • Ensure timely payment of services and support.
  • Provide financial accountability and security for the payment of funds to LSA on behalf of the family, and ensure appropriate accountability and acquittal.

The individual and their family will:

  • Meet with service providers to determine suitability.
  • Participate in the selection and recruitment of staff where required.
  • Actively participate in the plan development and review.
  • Engage in mentoring opportunities to increase skills in self-managed funding.
  • Utilise templates provided to monitor service delivery and expenditure.

Model C – Independent

This model provides the greatest opportunity for self-managed funding and has the least involvement from Life Stream Australia. (Max 2 hrs per month Host Coordination)
LSA will:

  • Publish the Services and Support Plan as designed by the individual and their family.
  • Ensure timely payment to the individual or family member’s nominated bank account.
  • Provide financial accountability and security for the payment of funds to LSA on behalf of the family, and ensure appropriate accountability and acquittal.
  • Maintain contact with the individual and their family (quarterly) regarding service delivery.

The individual and their family will:

  • Develop all aspects of their Services and Support Plan using the templates provided.
  • Recruit, train and performance manage employees.
  • Undertake and manage all legal and industrial aspects of employing staff.
  • Manage complaints or discrepancies with other service providers listed in the plan.
  • Submit necessary acquittal information to LSA for payment, using the templates provided.

In the event additional support is required to undertake the responsibilities listed above, LSA will charge an hourly rate for the Hosting Coordinator’s support ($72 per hour). A quote will be provided to the individual and their family at the time of the request and support can then be negotiated

Note: The above charges are inclusive of ongoing administrative charges.
