Host Provider Model

Life without Barriers has been offering individually tailored support to people with a disability and their families since 1996. We are experienced working in partnership with people to manage their own support. Our support co-ordinators will collaborate with the person to undertake a comprehensive assessment, describing their needs, wants and aspirations for the future. We will help develop a Person Centred Plan which includes strengths based active support to maximise independence and meet goals and objectives.

Where a person chooses, we will assist in the recruitment of appropriately skilled staff or help find a suitable agency to implement the person centred support plan. We will ask the person’s preferences in terms of the characteristics, skills and experience of their support team and work carefully to match each requirement accordingly.

The person with a disability can decide if they wish to receive additional co-ordination services such as help to arrange housing and health care. Where requested, our multi-disciplinary clinical support team can develop plans, procedures and offer training in providing more complex support. These services may be purchased directly from LWB, or we can support you to purchase these services elsewhere.

We recognise that needs and circumstances change. Through our monitoring and review processes our co-ordinators remain closely engaged with the person, ensuring the service provided is of a high quality and the individual is satisfied. During these reviews, we will provide an explanation of how the person’s funds are being used, and change the Person Centred Plan to reflect new goals and objectives.

Fee Structure

Life Without Barriers has a schedule of fees outlining what we provide and the costs. Costs vary depending on whether you self mange or self direct. Costs are also determined by the level of planning, facilitation and expertise for which you wish to engage Life Without Barriers, the type and number of service purchased.

A written estimate of cost, including the fee structure, will be personalised and provided to potential clients prior to entering into any agreement. Life Without Barriers charges a one-off establishment fee of $500. Administration costs are 6% for Self-Management and 12% for Self-Direction.

All other specialist supports and direct services that may be provided by Life Without Barriers will be negotiated with you.
