As a provider it is your responsibility to complete a Yellow Card (or Yellow Card Exemption) application on behalf of an applicant.
In consultation with the applicant, check whether they hold a current Blue Card:
As a person commencing work for a provider, it is your employer’s responsibility to complete a Yellow Card (or Yellow Card Exemption) application on your behalf and in consultation with you.
The provider must seek your consent to conduct the screening and must sight two forms of original identification to verify your identity, prior to lodging the application.
As a sole trader you must hold a valid Yellow Card before delivering services under the NDIS.
Any person who holds a current Blue Card (that is not suspended and is valid for 3 months or more), may apply for a Yellow Card Exemption and be exempt from paying any fees that would normally be associated with a Yellow Card. Please note, if the Blue Card is cancelled or expires, the Yellow Card Exemption will also be discontinued.
